Please provide only your name and way (phone number, email) to contact you for the purpose of making an appointment.  The Attentive Dentist, LLC uses this information to return your request for an appointment.  When you become a patient you may opt out of communication via text, email (gpatino@attentivedentist.com).  You may also do this by calling us at 862-397-4282.  Wix tracks city and state of visitors of the website.  This data is collected for analysis of where patients are coming from, no details such as ip addresses are linked to any personal information.  We are currently working on making the website more accessible.
